1. The email address you used to register.
2. The name, city, and state of the center you’re currently registered with.
3. The name, city, and state of the center you’d like to switch to.
Once we process your request, we’ll confirm the change. Remember, this change is a one-time adjustment, so choose your new center carefully! 🎳
